This collar provides plenty of neck support with the metal brace, and it really is firm. I use it for 1 and 1/2 hours per day.

I ordered a regular size-would size up next time just because of the surgical padding I had for the first couple of days post-op, but was also already on high end of measurements for the size I chose.
